Discovery Transitions Review
Discovery Transitions is an outpatient substance abuse treatment facility for adults in Van Nuys, California. Discovery Transitions is the outpatient component of the inpatient program at The Discovery House. It is open to those who have completed primary care at The Discovery House or elsewhere. The programs at Discovery Transitions offer a unique combination of peer support, recovery coaching, evidence-based therapy and 12-step principles.
Treatment and Staff
Treatment at Discovery Transitions begins with an initial assessment. For those who are a good fit, an individualized treatment plan is devised. Discovery Transitions provides dual diagnosis support for those with co-occurring disorders. The most structured level of care offered is Intensive Outpatient Program (IOP), with the option to step down to basic outpatient care after six weeks. Clients can also choose from a variety of stand-alone recovery services including sober coaching, group and individual therapy or psychoeducation sessions.
IOP clients attend about nine hours of group therapy and one individual session each week in addition to working with a sober coach. The outpatient level of care includes one three-hour group and sober coaching, with individual therapy as needed. Psychoeducation groups address coping skills, emotional intelligence, anger management and communication. The facility offers both gender-specific and co-ed groups in addition to a relapse prevention track.
The program employs LSWs, psychologists and CASACs. The staff members, most of whom are in recovery, also help residents get connected with 12-step groups in their area to help them pursue ongoing recovery.

Discovery Transitions serves the entire San Fernando Valley and Ventura County. Alumni are encouraged to return to the comfortable, welcoming facility to play pool, fellowship, share their successes and use it as their sober hangout.
