Horizons Family Centered Recovery Program provides outpatient and residential substance abuse treatment at Covenant Medical Center, a full-service hospital in Waterloo, Iowa, a mid-sized city two hours northwest of Des Moines. Covenant Medical Center is part of the Wheaton Franciscan Healthcare network, which began serving the residents of Iowa over 140 years ago.

The Horizons Family Centered Recovery Program has been in operation for over 30 years. Its outpatient offerings include a Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP) and a relapse prevention program. In addition to its outpatient and residential offerings, Horizons offers a detox program that lasts between two and 10 days, depending on the substance use disorder.

Treatment and Staff

The duration of each treatment track is usually three to five weeks, depending on a client’s progress. Clients can also step down to a less intensive track as they progress in their recovery. In the most intensive level, the PHP, clients attend group and individual therapy from 8 am to 4 pm Mondays through Fridays. Clients in this program may also board at the hospital. This makes their treatment similar to the residential program, but boarding PHP clients cannot receive medication from Covenant Medical Center.

Clients in the IOP attend group therapy Mondays through Thursdays, either from noon to 3 pm or 5 pm to 8 pm. Clients in the IOP and PHP also receive individual therapy at least once a week. Clients in the Relapse Prevention group, who’ve completed treatment before and have a risk or history of relapse, attend group therapy on Tuesdays and Thursdays from 5 pm to 7 pm. Relapse Prevention clients have individual therapy as-needed.

Groups have fewer than 15 participants and are led by Master’s-level therapists and addiction counselors. The treatment team is experienced treating co-occurring disorders and can refer clients to Covenant Medical Center’s behavioral health department for psychiatric care. Therapists and counselors use the Matrix Model and incorporate elements of the 12 steps. They encourage clients to attend 12-step and other peer support meetings such as Celebrate Recovery as often as possible.
