Derby New YorkThe Basics

The TLC Health Network is a multi-location provider of acute, primary and behavioral health care services in Southwestern New York state. TLC was formed in 2002 with the merger of Tri-County Memorial Hospital and Lake Shore Health Care Center in Irving, New York. Its Derby and Cassadaga locations—near Lake Erie—offer outpatient substance abuse treatment, including Medication-Assisted Treatment (MAT) with Suboxone and Vivitrol. TLC previously provided residential treatment for women in Irving, however, that facility closed three years ago.

Treatment and Staff

After an assessment, the treatment team meets to formulate a treatment plan and assign the client a primary counselor. Within a week, the client and counselor meet to discuss the treatment plan which includes a combination of group and individual therapy, with most clients attending individual therapy once a week and group therapy twice a week. The duration of treatment is progress-based, but most clients stay in treatment an average of six months. TLC encourages clients to attend 12-step or other peer support meetings while in treatment, though they’re not a requirement.

Groups are led by Master’s-level therapists and addiction counselors, including LMSWs and CASACs. Therapists and counselors use Motivational Interviewing (MI) and Trauma-Informed Care (TIC) to help clients improve their thought and behavior patterns. Topics include coping skills and relapse prevention as well as gender-specific issues.

Clients with co-occurring disorders are referred to an affiliated mental health clinic for additional therapy and medication management for their mental health diagnosis; however, TLC is currently developing a group for dual diagnosis support.

TLC has a physician who comes to its clinics on Mondays and Fridays to monitor Suboxone administration. Most clients receive two weeks worth of doses at each visit. TLC also has a therapy group dedicated to exploring opiate addiction.

Extras

TLC offers an adolescent substance abuse treatment group as-needed. It also administers the opioid-overdose reversal drug Narcan. Clients can receive case-management services to access referrals other agencies and treatment programs through its clinics and Lake Shore Health Care Center.

With customized long-term treatment plans, low fees and aggressive initiatives to help opiate addicts, TLC Health Network’s outpatient programs are highly responsive to the changing needs of their communities.
