Located in Fall River, Massachusetts is Transition House, a residential treatment facility under the umbrella of Steppingstone, Inc. It’s intended for adult men and women who have been disabled and struggle with chronic homelessness and co-occurring disorders. Treatment includes individual and group therapy, relapse prevention, psychoeducation and assistance in permanent housing placement. Transition House offers programs that are based on the 12-step philosophy along with evidence-based therapeutic approaches.

Accommodations and Food

The 10-bed facility is a large, three-story Victorian style home located in a safe and quiet residential neighborhood in Fall River. The bedrooms are private and residents share the six bathrooms, along with a large kitchen, dining room and living room with TV. There is also a spacious backyard area, offices and meeting spaces. The residence is near state parks and wildlife refuges, in addition to shops and grocery stores and the local college. During their stay, clients are able to seek out employment or continuing education in the community.

All food is provided by Steppingstone, Inc. and clients trade off preparing meals for each other daily. The meals aim to be healthy and nutritious.

Treatment and Staff

The program offered at Transition House is a combination of therapy, case management and education, in addition to daily AA/NA meetings. The program is structured to give clients skills training along with addiction treatment and medical care. Every client receives a full medical assessment prior to beginning treatment and if necessary, medication management and dual diagnosis support can be facilitated. On average, clients stay at this facility for three to six months, though longer stays can be arranged.

Treatment methods include CBT, DBT and trauma-informed care. There are several group sessions a day that address past traumas and offer relapse prevention and anger management techniques. Clients also have weekly individual therapy to discuss and assess treatment goals. There are health and wellness classes and independent living skills training, including employment assistance, permanent housing after discharge and referrals to receive additional care for dental, medical and psychiatric.

The staff of Steppingstone Inc. and Transition House consist of medical and clinical addiction specialists like LCSWs, psychiatrists and substance abuse counselors. Additionally, there is a residential staff that is available 24 hours a day.

Family counseling and aftercare services include weekly group therapy and alumni programs.
