Rules and Regulations
This transitional living environment seeks to not only help treat the symptoms of mental health conditions but also to heal the “whole person.” As such, clients go through a comprehensive evaluation that helps the team assess the most effective individualized care plan customized to meet the resident’s specific needs, goals and situation. A recovery plan is then designed to both treat and empower the client.
The team here offers a holistic approach to care that integrates a number of therapeutic methods and services including skills training, recreation, counseling and transportation. Clients engage in individual therapy and have access to medication management to treat mental illnesses.
The facility encourages residents to overcome personal obstacles and engage with life again. As such, life and social skills training is a major component of the Northwest Transitions program. Residents spend a lot of time socializing with staff mentors and peers in addition to gaining new job skills.
